for tooth-colored posterior restorations

Description and Operation Manual:

Set Composition

The UNIVERSAL SET is a nine-piece preparation and finishing set, consisting of 3 cylindrical 80 um preparation diamond burs (PD) marked in blue, 3 cylindrical 25 um finishing diamond burs for com-posite cavities (CF), marked in white and 3 conic 25 um finishing diamond burs for inlay cavities (IF), marked in white.
The 3 bur categories PD, CF and IF come along in sizes "large", "medium" and "small". The finishing burs Match the preparation burs in size. The inner fine angles automatically take on a round shape during the finishing process. The conic inlay finishing burs provide a cavity opening of 2 x 4°.

Indications

For directly layed composite restorations or tooth-colored inlay in Class I (0) and Class II (M0, OD, MOD, Slots) cavities in course of the primary treatment of carious lesions or the replacement of restorations.

Goals

Rationalized cavity preparation during the filling therapy with tooth-colored restorations in premolars and molars.
Maximum preservation of dental hard tissue during the preparation of cavities. Production of cavities which are ideal for the use of the adhesive technique, characterized by a round, soft cavity design and sharply cut, right-angled, fracture-free cavosurface margins.

Practical Procedure

Principally, the cavities should be kept as small as possible taking into consideration the form of the cavity, the accessibility and the occlusal load.
Demineralized but still structured enamel should not be removed. A layer of healthy dentin, at least 1 mm thick should support the enamel margins.
Caries and restoration remnants should be excavated by undermining the cavity margins.
Before the finishing procedure, the near exposures should be coated with a Ca (0H)2 liner. The exposed dentin should be sealed using a glass ionomer cement liner or an equally good base. Undercuts should be blocked out in excess using a build-up base, presently also preferably a glass ionomer cement.

Bonded Resin Restoration

For the finishing of the cavosurface margins and the shaping of parallel walled cavities, the cylindrical 2S L m composite finishing diamond burs are used.
The risks of marginal enamel fractures are reduced by dulling the occlusal and axial cavosurface margins preferably using fine and flexible discs, in case less sophisticated restorative techniques, than the three-sited light curing filling technique (Quintessence Int. 18: 125-134, 1987) are chosen

Bonded Tooth-Colored Inlays

The cavosurface margins should be finished and the cavity reshaped using the 25 gm inlay finishing diamond burs. During this procedure, the finishing burs must not be inclined to the buccal or lingual tooth surface in Order to prevent an unnecessary enlargement and occlusal opening of the cavity.  The finishing burs have to be kept strictly parallel to the long axis of the tooth resul-ting to buccal and lingual cavity walls diverging at an angle of 2 x 40.
The cavosurface margins are left sharply cut and right-angled. Dulling or bevelling of the cavosurface margins should be waved to prevent an undesired enlargement of the resin-based cement  layer. The stress exposure of the enamel is mini-mal due to the inlay technique. With cement layers <150 um no marginal enamel fractures are caused by contraction stress even in unbevelled enamel margins.

  • Bonded tooth-colored inlays: maximum conservative inlay cavities with a bilateral opening angle of 40 and persistent rounded inner line angles due to the use of the conic 25 um finishing diamond burs UNIVERSAL 1-, 2- or 3-1F.

  1. Do not remove deminerafized, but still structured enamel; enamel margins should be supported by 1 mm sound dentin

  2. Undermining excavation of caries and removal of restoration remnants

  3. Covering of near exposures using a Ca(OH)2 liner

  4. Placement of build-up base using a glass ionomer cement

  5. Bonded restoration: finishing of the cavity leaving the walls parallel: no further widening of the cavity
