E q u i p m e n t This set of burs was assembled in collaboration with: Dental School, Department of Operative Dentistry, at University of Bern
Tooth caring bur (tc-b) for perfect preparation margins. Indications Lowering gingival floors of approximal cavities and shoulders of crown preparations without damaging neighbouring tooth surfaces.
Armamentarium
Cutting approximal boxes and shoulders of crown preparations using conventional instrumentation and methods will lead to iatrogenic of adjacent tooth surfaces in 75 - 95% of the cases. Follow-up studies over up to 7 years after initial therapy indicate that these iatrogenically injured surfaces needed restoration significantly more frequently than untouched surfaces. Tooth caring burs (1) allow lowering gingival floors of approximal cavities and shoulders of crown preparations leaving them with a perfect finish without touching neighbouring teeth even in narrow approximal contact situations.
The tooth caring-armamentarium consists of a series of front end cutting diamond burs with a slightly recessed cutting face (2). They come in two working diameters (1.0 and 1.3 mm) and two diamond grits (40um and 25um). The larger of the two is for universal use whereas the o 1.0 mm bur is especially helpful for mini preparations. The 40um grit is suitable for careful cutting while the 25um will produce a smooth finish.
The bur can be tilted up to 20° without cutting into the adjacent tooth. The method presented will cause significantly less injuries to neighbouring tooth surfaces. Cumbersome 'protection" of adjacent teeth with matrix bands is unnecessary thus allowing an unobstructed view onto the fein of operation. For the removal of the remaining enamel fin and finishing of the margins especially in the area of transition from the cervical to the approximal portion of the cavosurface margin (of non bevelled preps for ceramic-, amalgam- and composite restorations) we recommend to use the distally bent diamond-coated Cavishape(R) files in the new vertically oscillating EVA-hand-piece (Prepcontrol(R) with a reduced stroke of now 0.4 mm, formerly 1.5 mm). For small bevels of approx. 0.3 mm Bevelshape(R) files may be more appropriate. Procedure 1. Lowering gingival floors of approximal cavity preparations
1.1 Gain access to the carious process using adequate rotating instruments of appropriate size (e.g.. Piccolo(R) diamond burs) (3).
1.2 Remove the enamel fin with a hand instrument or the non rotating diamond bur (4).
1.3 Lower the gingival extension with the 40um- and finish off with the 25um tooth caring bur respectively (5).
1.4 Finish off vertical cavosurface marine ( including the round gingivo-axial corner transition) with Cavishape(R) (6,7) in the lockable Prepcontrol(R) contra-angle handpiece (stroke 0.4 mm, transmission 1:1 ). When a small bevel is needed the Bevelshape(R)-file are suitable (8).
1.5 Overview for the procedure (9): lt has been shown, under daily working conditions in the dental office, that the procedure outlined here and its armamentarium significantly reduces preparation trauma to adjacent teeth.
2. Lowering a shoulder for a crown The tc-burs can also be used very effectively for lowering crown shoulders to the desired depth when doing crown-and bridgework ( 10, 11).
