E q u i p m e n t Rootshape®-files for careful rootplaning
Treatment of Rootsurfaces Elimination of the soft, microbiological plaque from the root surface is today regarded as the essential aspect of the periodontal therapy. The usual instruments used for the elimination of subgingival tartar followed by rootplaning, are sealers and curets. A systematic therapy of the rootsurfaces followed by a local careful treatment, is regarded as the established procedure. The Roothshape^set represents a novelty for the treatment of periodontitis. These new file shapes in combination with the free movable (61 LA) and the fixed (61 LR) EVA-head by KaVo, are suited for the closed as well as the open (surgical) treatment of root surfaces. The Root-shape® (1) file has a flexible proximal blade and is available in two lenghts. The working-end is spoon-shaped and, on its convex side, diamond coated with a 40, 15 µm or 2-4 µrn granulation. Due to the oscillating file movement and the fine design of the flexible instrument, it is possible to treat rootsurfaces even in places that are morphologically difficult of access. The instruments were developed in cooperation with the Department of Operative Dentistry of the University of Bern. Indications 1) Elimination of supra- and subgingival plaque 2) Rootplaning 3) Odontoplasty 4) Finish and prepolish of restorations Applications Which of the two EVA-heads is used, depends on the required application. The head 61 LA allows free rotation of the file and thus an automatic adaptation of its working-end on the rootsurface. The fixable head 61 LR is prefered when a defined removal of a specific amount of substance is desired (e. g. for odonto-plasty). The files should be applied in combination with the blue handpiece, at a speed of rotation of between 10000 and 15000 rpm. Permanent water cooling is important. 1. Plaque elimination
When treating recall patients, subgingival plaque can be removed easily with the Rootshape® files (2-4 µm), that is ergono-mical and careful of the tooth substance and gingiva (2). The substance loss caused by the 2-4 µm diamond coated file is, in comparison to the 15 µm or the 40 µm file or the curet, very little.
2. Rootplaning
A slight pressure and an overlapping procedure should be applied when rotsurfa-ces are treated with the Rootshape® files. The working movements can be directed horizontally, vertically or diagonally to the tooth axis depending on the accessibility. Due to the diamond-covered working-end of the file (15 µm) lying flat on the root-surface, there is only little loss of the tooth substance. Application of the 40 µm diamond coated file should be followed by the 15 µm file in order to obtain a micro-morphological smooth rootsurface. The substance loss, when applying a force of 160p, is comparable with that caused by curets. The smooth micro morphological root surface can be judged by the SEM picture (1 enlargement lOOx). ![]()
Due to its thin leaf and flat working-end the Rootshape® file can reach the bottom of deep periodontal pockets. The flexible file follows the root, passing the crown camber. A wide shore up between tooth and gingiva is avoided (4). Owing to the file's flexibility it is not possible to apply too high a pressure. Long files are available for deep probing depths. The Rootshape® set contains a delicately formed file which is especially suited for morphological problem zones like the narrow interdental space or deep bone pockets. The spoon shaped end allows furrows and retractions in root bifurcations to be reached (5). For open surgical periodontal treatment (6) the EVA head (61 LA) can be equipped with a "sprayclip" in order to connect a cooling system with sterile cooling-liquid. The cooling-liquid is led between file and root.
3. Odontoplasty When carrying out morphological corrections of narrow furcations or rill-shaped contractions with a 40 µm diamond coated Rootshape®' this should be finished with a 15 µm file. 4. Working out and prepolishing restorations ![]()
Finishing restorations with rotating instruments can be difficult especially in the in-accessable interdental area. For that job this set of files is very suitable. Fig. 7 shows the finishing of a composite restoration on tooth 23 (compare tooth 22). The tooth is periosurgically prepared and isolated with a rubber dam. Kidney shaped contractions or rills in the crown-root area are problem zones as soon as fillings have their margin in that area. The Roothshape® set of files (40 µrn and 15 µn) is well suited for the elimination of overhangs and the finishing of filing margins (8). A further application is the finishing of margins after cementation of adhesive in- and onlays with the 15 µm Roothshape®. Order List
