E q u i p m e n t A supplement to the mechanical-Instrumental periodontal treatment. The only Perio Set created by Prof. K.H. Rateitschak, University of Basel One of the most important aspects of periodontal therapy is the cleaning of the root surface. Supra- and subgingival plaque as well as calculus must be entirely removed. This cleaning must encompass the removal of the cementum layers on the root surface containing endotoxins. Only under these conditions can a healing or regeneration of the periodontal tissue following a periodontal treatment be expected. The best instruments for this time-consuming root cleaning and planing are still the scaler and curette. In advanced periodontal cases the bi- and tri-furcation of the multiple root can be opened. In addition, indentations and irregularities in the roots will be exposed through bone loss. These small recesses are however not accessible with a curette. A perfect treatment is impossible. lt is especially in these cases that a PERIO SET will facilitate and improve the treatment of the root surface. The rather coarse (75 um diamond grain) instruments are designed for odontoplastic enlargement of furcations and concavities so that accessibility to these areas with the curette and finer grained instruments can then be accomplished. The 40 um diamond (yellow band) is designed for removing calculus and other deposits. lt will also remove superficial cementum layers. The fine grain 15 um diamond (red band) should be used for a final planing of the entire root surface and produces a result comparable to that which is obtainable with a curette.* These instruments may be used for the above-mentioned tasks during closed curettage as well as for open flap surgery (modified Widman flap) during which the root surface is exposed and visibly treated. The PERIO SET cannot replace curettes and scalers: but is designed to ease root cleaning and planing which, while often technically difficult, are a crucial step in periodontal treatment. Use of the finest diamond bur (15 um). Left: a groove in the surface of the left upper first premolar is smoothed with this instrument; Right: the same instrument used on the palato-mesial part of the root. Root surface is smoothed with a "blue" anglepiece used at medium speed under continuous cooling with a physiologic solution.
