E q u i p m e n t Cavishape®-file for perfect cavity preparation
Instruments Several studies have shown that in at least two thirds of the preparations the adjacent teeth are damaged. This trauma increases caries progression and the perceived need for restorative therapy of the adjacent teeth. Protection during cavity preparation by matrix bonds is possible but seldom carried out. The margin trimmer which does not damage the adjacent tooth leaves a rough margin.
The Cavishape®-file with its ideal form and in combination with the newly developed adjustable EVA head (1) (KaVo, 61 LR; stroke length 0.4 or 0.8 mm) permits perfect and risk-free finishing of the cavity border. It has been shown in vitro and in vivo that this new instrument is superior in finishing cavity borders. Cavishape® is a highly flexible file which is terminally curved. It is available in 25 um and 40 um diamond grains and in a width of 1.0 and 1.5 mm (2). The somewhat stiffer and distally bent new Cavishape® R-files are especially suitable for cutting cavities and finishing the cavosurface margins for tooth coloured restorations of the "adhesive" (resin luted) type. However, they must be used with the new EVA contra-angle head (Prepcontrol®) with increased stroke (0.8 mm) and built in shockabsorber. Indications Finishing of all proximal class II borders 1. Slots and other cavities 2. Adhesive restoration (ceramic, composite, CEREC®, CELAY®) Applications 1. Finishing of slots and other cavities (step-by-step) 1.1. Gross preparation of cavity with Piccolo diamond bur of adequate size 1.2. Finishing of cavity margins
1.3. Thin enamel wall remains (3,4)
1.4. Finishing of proximo-cervical border with Cavishape without injuring the adjacent tooth (5,6)
1.5. REM: a finished cavity (7); vertical margins without enamel fracture. Right picture: clinical example (8)
2. Adhesive restorations The adjustable EVA-head with the Cavishape®-file finishes also the borders of ceramic, composite- in/overlays preparations.
It is possible to finish the axial border and the curvature (9, 10) in one step, providing the file is held apically enough (11). There is no danger of injuring the tooth to be prepared.
